(Sorry for incorrect thread title, forgot to edit after further testing)

If you die at any point after riding a Demolisher (whether you're still on it or have hopped off), the game throws this error:

The mod Worm Rider (0.9.4) caused a non-recoverable error.
Please report this error to the mod author.
