Oh, can't believe I didn't see this until now, but your screenshot is showing the Cybernetic Combinator from the Project Cybersyn mod itself.

The combinator from this mod is called "Cybersyn constant combinator".

(Didn't get around to trying to replicate this until now and realised this.)
