Bugfixes:
- Fixed machines emitting smoke while idle. Boilers, furnaces, and burner mining drills now only smoke while their status is actually "working" (burning fuel), instead of any time the entity was active. An idle or unfed boiler no longer puffs smoke.

Bugfixes:
- Removed invalid smoke prototype keys (wind_speed_factor, vertical_speed_slowdown) that Factorio 2.0 silently ignored, so smoke now only uses supported fields.
- Removed a duplicate affected_by_wind assignment in the smoke builder.
- Fixed rocket and explosion smoke not appearing. Rocket launches now emit smoke through on_rocket_launched, and destructive entity deaths now emit blast smoke directly instead of relying on on_trigger_created_entity, which rarely fired for base entities.

- Explosion and death smoke is near-player gated and scales with entity size to stay UPS-conscious.
- Enemy (unit) deaths emit smoke only occasionally so large battles do not flood the map with smoke.
