Possible issue with Pyanodons compatibility, specifically the long mechanical inserters
For some reason, it drops a standard (electric) long inserter when you break it, even if you have the downgrade when broken option on. It's also unlocked alongside the long inserter in research and seems to be called 'burner-long-inserter' in the code? (not sure if that last one is intentional?)

Costless upgrade is on, fuel leech is on, all other startup settings are off

All three issues should be fixed in v5.2.7:

• Long Burner Inserter (named Long Mechanical) drops as burner inserter when the drop-as-self setting is disabled
• Long Burner Inserter is enabled at start, alongside Burner Inserter, instead of unlocking with Long Inserter
• (unmentioned) PyStack Inserter is properly sorted after the Bulk Inserter (when feature flags are not enabled)

It's called long-burner-inserter internally because that's what it is lol. PyCoal renames the burner-inserter to Mechanical Inserter, and long makes a long version based on the internal name, using the localized name so it matches whatever the burner is called.
