1. Please move inserted warp modules (and "fuel") to the bottom side screenshot: https://i.imgur.com/xBuNx2L.png it will make portal look a whole lot better
2. You could change module icons to planet icons, though they might be bit small.
3. Allow for remote interacting with portal
4. Add warp modules for modded planets (at least the big popular ones), or universal warp that could be used for modded planets (though if thats the case they shouldn't be too heavy for rockets).
5. Optional. You could add teleporting energy cost on top of the "fuel" that can be edited it settings.

All in all mod works great and i was searching for something like this.
