If you revive an underground belt with the belt in your hand, it goes in the way the one in your hand is facing, instead of the way the ghost image is facing.
I also have this problem. As far as I can tell, game logic on which way the belts should face is messing with the mod.
Yes, this is quite annoying - pre-bots I have some lane balancer blueprints which make use of "unusual" underground belt orientations, and I have to manually fix every one of them.

The following change to 'scripts/reviver.lua' works with Factorio 1.1.36:
Whole file reviver.lua:
Simply replace the file in the mod '.zip'.
